Insurance appeal guide
What supports a medical-necessity appeal?
A medical-necessity denial questions whether the requested care meets the plan’s clinical requirements. A focused appeal connects those requirements to the documented facts and relevant evidence.
Start with each stated reason
Separate the reasons listed in the notice. One may question prior treatment, another the severity of the condition, and another the evidence for the proposed service. Identify the plan policy or criteria cited and the version used when available.
An appeal should address each relevant point directly. A guideline or study can support a medical argument, but it does not by itself establish coverage under a particular plan.
Build a concise evidence map
| Question | Potential documentation |
|---|---|
| What service was requested, and why? | Treating-clinician notes and the request itself |
| What clinical facts relate to the criteria? | Relevant tests, findings and chronology |
| What alternatives were tried or considered? | Treatment history and documented rationale |
| What evidence supports or limits the request? | Relevant clinical guidance and published studies |
The documents should explain the actual patient circumstances. Avoid asserting a treatment failure, contraindication or test result that the record does not establish. Ask the treating clinician to clarify gaps rather than inventing a stronger history.
Experimental or investigational denials
When the plan questions the evidence base for care, review the specific determination and the evidence relevant to the proposed use. Study population, intervention, outcomes and limitations matter. Regulatory status, publication or professional use alone may not resolve the plan’s stated reason.
A physician assessment may find medical support, missing documentation or an unfavorable limitation. EUC’s assessment is a separately scoped service; appeal preparation and submission are not automatically included.
Confirm the next step
The denial notice and plan documents control the available appeal route and filing instructions. An unresolved medical judgment dispute may be eligible for external review, depending on the plan and applicable process. Confirm eligibility and deadlines before assuming another review is available.
